CERTIFICATION APPLICATIONS
CCTSW - Certified Clinical Transplant Social Worker
To apply for CCTSW Credentials, complete the application checklist below.

CCTSW APPLICATION CHECKLIST
(1) Application Form
- Complete and submit the Application form.
- You will not be able to save and return to the form. Please be prepared to complete the entire application form at one time.
- Please note the application form will require you to upload the following documents. Please have them ready to upload prior to starting the application.
- Your current social work license
- License must be at the highest level of social work licensure available in the applicant's state/province/country of practice. This is a clinical certification.
- Ensure that the license includes the expiration date.
- Attendance certificates for 20 solid organ transplant specific continuing education (CEUs) within five years of applying.
- Please submit attendance certificates, not a transcript of attendance.
- Please ensure that contact hours are noted on each attendance certificate, or proof of hours that specifically notes the length of the program and attendance if no certificate is available. If contact hours are not noted, the course will not be counted toward your credentials.
- If possible, please combine all CEU certificates into one document prior to uploading.
(2) Manager/Supervisor Attestation
- Please have your manager or supervisor complete the online Manager Supervisor Attestation form.
- Please have TWO professional references complete the online Professional Reference form.
- At least one reference should be from a social work colleague.

- Ask your graduate school to email your transcript to credentialing@stsw.org.
- Please note the transcript must come to credentialing@stsw.org directly from the graduate school, not from the applicant.
- Payment can be made online by credit card or by check. Instructions are available for both on the Credential Payment page.

To apply for CCSW-MCS Credentials, complete the application checklist below.

and- Attendance certificates for 5 MCS specific CEUs within five years of applying.

- Please ensure that contact hours are noted on each attendance certificate, or proof of hours that specifically notes the length of the program and attendance if no certificate is available. If contact hours are not noted, the course will not be counted toward your credentials.
