Description:

Under the supervision of a social worker who meets state required standards, the social worker in this position opening will fulfill the role of a Donor Social Worker.

The Donor Social Worker conducts comprehensive psychosocial assessments, individual psychotherapy, treatment plans, and support, including coordination of discharge planning for potential kidney and liver donors through all phases of care including pre-donation, donation, and post-donation.

The Donor Social Worker collaborates with the multidisciplinary transplant team, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within the Section of Social Work and the Department of Nursing.

Social worker with current licensure at the clinical level and eligible to practice independently. An MSW degree is also required from an accredited Social Work program. Candidates must have an advanced knowledge of mental illness, personality development, knowledge of the types of treatment, and skills in conducting individual, group, and family therapy. Must have knowledge of adjustment to medical diagnosis’ and chronic disease. Strong communication skills are needed to interact with patients, patient’s family members, multi-disciplinary team, community agencies, and other resources. Must also have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and in written form. Licensure to practice independently at the clinical level as required by applicable state requirements. Valid driver's license required for specific specialty areas.

Preferred: Knowledgeable about living organ donation, transplantation, medical ethics, and informed consent.

Pediatric Transplant Social Worker- Liver and Lung Transplant Programs

Job Description:

This is a 40 hour 100% on site pediatric transplant social work position in the lung and liver, intestine, and multi-visceral transplant programs at Boston Children’s Hospital. The role includes a range of clinical social work and case management services in the inpatient and outpatient setting.  This social worker will care for patients in the transplant program during all phases of transplant including transplant evaluation, transplant admission, and transplant follow up.

Key Responsibilities:

·         Psychosocial assessment as part of transplant candidacy for the program

·         Counseling and crisis intervention

·         Facilitation of the use of hospital and community services

·         Resource information and referral, and program development

·         Psychosocial consultation to multidisciplinary team

Minimum Qualifications:

Education:

-Master’s degree in social work

Experience:

·         2 years plus experience as a licensed Social Worker

·         Experience in medical environment; bereavement, child protection issues preferred

·         Flexibility and ability to work in a fast-paced, dynamic environment

·         Excellent communication skills are necessary.

Transplant Psychosocial Coordinator

The Transplant Psychosocial Coordinator plays an integral role in assisting transplant patients with the critical function of coordinating psychosocial needs of candidates, recipients, living donors and families as well as financial planning for their transplant surgery and future medical needs. The Transplant Psychosocial Coordinator is also a key member of the Transplant Center selection committee and must provide up to date and accurate input on patient psychosocial status and financial clearance for transplantation.

Duties and Responsibilities:

  • The psychosocial evaluation of potential living donors and recipients
  • Substance abuse evaluation, treatment referral, and monitoring
  • Individual counseling
  • Crisis intervention
  • Patient care conferences
  • Patient advocacy
  • Transplant team building
  • Patient and family education
  • Death, dying, and bereavement counseling
  • Referral to community services
  • Department meetings including staff and quality meetings
  • Participation in organ donor awareness
  • Outreach efforts to increase referrals and communication to the transplant program from referral sources.
