Abdominal Transplant Social Worker (LCSW) , Toledo Ohio

Primary Function/General Purpose of Position

LCSW completes in-depth assessments of patients in order to evaluate candidacy for abdominal organ transplantation.  LCSW provides counseling, support, and community resource referrals to aide patients/families in preparing for transplant.  LCSW follows patients pre- and post-transplant to assist with resolving psychosocial issues and lifestyle changes that improve a patient’s post-transplant success.  LCSW works closely with multi-disciplinary transplant team to assess inpatients in order to identify appropriate discharge plans.  LCSW provides assessment and intervention planning with patients experiencing substance abuse and/or mental health issues.   LCSW will participate in community outreach and patient education related to transplant.

Minimum Qualifications:

Licensure/Certification/Registration:   LCSW required.

Education:  Master’s in Social Work from an accredited program required

Experience:   At least three years post-Master’s experience preferred, preferably in a transplant setting.

Transplant Social Worker, Kidney/Pancreas

Location: UMMC (University of Maryland Medical Center) Downtown Campus – Baltimore, MD

Multiple opportunities available for social workers licensed at the LMSW (Social Worker I) or LCSW-C (Social Worker II) levels. Positions currently open in kidney/ pancreas transplant. The Transplant Social Worker will work on an interdisciplinary team to provide social work services to transplant patients across the continuum of care.

Responsibilities Include:

  • Completes transplant psychosocial assessments, recommends interventions, and monitors progress as needed to assist in preparing patients for transplant.
  • Provides clinical services including brief counseling, psychoeducation, facilitating support groups, behavioral interventions, and crisis intervention.
  • Participates in discharge planning and provides referrals to community supports.
  • Reviews patient chart for pertinent information; maintains appropriate documentation and communicates with the interdisciplinary team.

Required Qualifications:

  • Master’s Degree in Social Work (MSW)
  • Licensed as LMSW or LCSW-C in the state of Maryland

Preferred Qualifications:

  • Two years social work experience in an acute hospital setting. This may include internships and paid work experience.
  • Transplant experience highly preferred.
