Transplant Social Worker, Kidney/Pancreas

Location: UMMC (University of Maryland Medical Center) Downtown Campus – Baltimore, MD

Multiple opportunities available for social workers licensed at the LMSW (Social Worker I) or LCSW-C (Social Worker II) levels. Positions currently open in kidney/ pancreas transplant. The Transplant Social Worker will work on an interdisciplinary team to provide social work services to transplant patients across the continuum of care.

Responsibilities Include:

  • Completes transplant psychosocial assessments, recommends interventions, and monitors progress as needed to assist in preparing patients for transplant.
  • Provides clinical services including brief counseling, psychoeducation, facilitating support groups, behavioral interventions, and crisis intervention.
  • Participates in discharge planning and provides referrals to community supports.
  • Reviews patient chart for pertinent information; maintains appropriate documentation and communicates with the interdisciplinary team.

Required Qualifications:

  • Master’s Degree in Social Work (MSW)
  • Licensed as LMSW or LCSW-C in the state of Maryland

Preferred Qualifications:

  • Two years social work experience in an acute hospital setting. This may include internships and paid work experience.
  • Transplant experience highly preferred.

About the Job: This position is for a pediatric Heart Transplant Social Worker.  The transplant social worker provides comprehensive social work services to patients and their families through all phases of the transplant process.  The transplant social worker will follow the patients and families from the initial referral to the transplant team through the transplant process and for the duration of the post-transplant period.  The social worker conducts comprehensive psychosocial assessments, provides patient education and emotional support, makes appropriate referrals to community resources, and coordinates patient care with members of the multidisciplinary transplant team.

Required:

May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess and treat emotional mental health issues. The individual will also be able to diagnose under the direct supervision of a LCSW social worker. This LMSW position will assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required.

Qualifications

Master's degree in Social Work. One year of current experience in a clinic, hospital, medical setting, or healthcare field preferred. Must have knowledge of adjustment to medical diagnosis and chronic disease. Strong communication skills are needed to interact with patients, family members/ care givers, providers and allied health staff. Must have psychosocial diagnostic skills and ability to coordinate transition of care. Active license through Arizona State Board of Behavioral Health Service Examiners; Licensed Master Social Worker (LMSW) required.

Transplant experience preferred.
